Form example with PHP

Hi,

I want to use the <form> tag , but when i submit the data i want to take the results with a php script. In Laszlo documentation there is an example with a jsp script, but since i don't know the language i can not use it. Does someone have an example like this with php.
I would be gratefull since my thesis is on laszlo and i am using php ang mysql.
